The Pharmacy Disaster Support Center is a service administered under contract by Pharmacists Mutual Insurance Company for the National Community Pharmacy Association Foundation.
The data collected during the enrollment process with regards to the Pharmacy Disaster Support Center will help accelerate the coordination of customer-specific aid to help you strengthen your disaster preparedness, response, and recovery procedures. To that end, the Pharmacy Disaster Support Center, Pharmacists Mutual Insurance Company, and the National Community Pharmacy Association Foundation, will consider the information you provide us to perform these services as personal identifiable information.
Such information may include, but may not be limited to, your company name, street address, whether you are a Pharmacists Mutual member, whether you are a National Community Pharmacists Association member, pharmacy license number, state board contact information, drug whole seller account number, utilities company contact information, computer vendor information, and landlord information.
The Pharmacy Disaster Support Center will only collect the information you are willing to share and we will only inform the individuals you direct us to inform. We will not share or sell this information we collect from you in providing this pharmacy disaster support service.
